Download PDF Document  Download MS WORD DocumentSAMPLE FORM -- Notice of Intent to Use IDEA Funds for Instructional Support Services (Early Intervening) - If your district decides to use IDEA funds under this provision, please contact the Special Education Planning and Policy Development Office. We will provide technical assistance to districts that elect to use funds under this provision. The form provided is a SAMPLE form. The request to use funds under this provision is completed as part of the Fund Code 240 (Federal Special Education Entitlement grant) and/or Fund Code 760 (ARRA-IDEA grant) electronic workbooks.
