Release of Information Forms

The Release of Information form is provided in English and 14 languages in audio, PDF, and Word format. Programs should be aware that accurate viewing and printing of the Word translations requires that the appropriate character sets are installed on the user's computer. If you have any question about the accuracy of a Word file, compare it with the PDF file. When in doubt, use the PDF file.
